Certificate confirming that Marta Józefa Olchawa (born Bauer) passed her diploma exams and was awarded the title of Doctor of Medicine, issued by the Medical University in Warsaw on 15 July 1963 – print on a single page of a paper sheet, filled in with handwriting and typescript, in Polish. In the top left corner, there is the name of the issuer: “MEDICAL UNIVERSITY | IN WARSAW | FACULTY OF MEDICINE DEAN’S OFFICE” and filing number: “106/6515/63”. In the top right corner, there is the place and date of issuance. At the top of the page, in the centre, there is the document title, followed by the main body below. At the bottom edge, in the centre, there is a round red stamp of the Medical University in Warsaw. Underneath the certificate, in the centre, there is information on the template: “Galanteria. Wolska 19. Zam. 457 n. 3000”. On the reverse, in the top left corner, there is a stamp of the State Notary Office in Warsaw certifying that the document is a true copy of the original. | Text by Monika Harchut
