Certificate of training in internal medicine, first degree, issued to Marta Olchawa (born Bauer) by the Health and Welfare Department of the Municipal Office of Warsaw on 11 November 1975 – print, single-page, on a blank paper sheer, filled in with a typewriter, in Polish. In the top left corner, there is the name and address of the issuer: “MUNICIPAL OFFICE | OF THE CAPITAL CITY OF WARSAW | HEALTH AND WELFARE DEPARTMENT | Warsaw, 81 Gen. K. Świerczewskiego Avenue” and filing number: “ZPS.XVIII-200-55/75”. Below, on the right, there is the place and date of issuance, document title, and its main body. Underneath, there is a round stamp of the Municipal Office of Warsaw and the name stamp of Jan Płoński M.D., Chief Provincial Physician, with his hand signature.
