Diploma of graduation from the Faculty of Medicine issued for Marta Olchawa (born Bauer) by the Medical University in Warsaw on 2 December 1963 – print on two leaves in the form of a bifolio, filled in by hand, in Polish. It comes with a black cover. On the front page of the cover, in the centre, there is an image of a crownless eagle and document title: “MEDICAL DOCTOR’S | DIPLOMA”. On the left page of the unfolded bifolio, there is a photo of Marta Olchawa with her hand signature underneath. The photo is stamped with a round red stamp of the Medical University in Warsaw. At the bottom edge of the page, there is information on the print of the template: “Mz/AM-1. PWHDA. Zam. Nr 3237/Wa/CWD | PDA. 1333-62 3000 leg. piśm. b/drz. kl. III/120 g. A6X2”. On the right page, at the top edge, there is the name of the school: “MEDICAL UNIVERSITY | IN WARSAW”, number: “L. 4362/6515/63” and document title: “DIPLOMA”, with the main body of the diploma below. Underneath, there is a round stamp of the Medical University in Warsaw and signatures of the Dean and the Rector. | Text by Monika Harchut
