Doctor’s oath made by Marta Olchawa (born Bauer), dated 6 January 1964 – print on a blank paper sheet, in Polish. At the top edge of the page, there is the document title: “DOCTOR’S OATH”. Underneath it, there is the main body of the oath, hand-signed by Marta Olchawa below. Under the main text, on the left, there is information on the place and date of signature: “Warsaw, on 6 January 1963”.
