Card (ausweis) No. 1673 belonging to Halina Allinowa, confirming membership in the Chamber of Health in the General Government, Warsaw Region, issued on 29 May 29 1942 in two languages – German and Polish – by the German administration office Gesundheitskammer in Generalgouvernement/Chamber of Health in the General Government in Kraków. The document is a bifolium, on the first side of which there is an attached form of instruction with the round stamp of the Chamber of Health in the General Government and the date of validity of the card until 31 October 1944. On the second page of the document is the identity photograph of Halina Allinowa and the round stamp of the Health Chamber of the Warsaw Region, also stamped on the photograph; underneath is Allinowa's signature. On the third page, Halina Allinowa's personal details are typed in, underneath which is a round stamp with the inscription: "Gesundheitskammer im Generalgouvernement f. d. bes. poln. Gebiete". Next to the seal is the signature of Dr Kroll SA - Sanitätsoberführer. On the fourth page of the document there is an acknowledgement of the correspondence of the photograph from page two; underneath the content there is a round stamp of the Health Chamber of the Warsaw Region and an illegible signature of the Health Chamber's representative. | MH
