Licence to practise as a medical doctor – document issued by the District Medical Council in Warsaw to Marta Olchawa (born Bauer) on 30 December 2002. It is an official document in the form of a 12-page booklet, in Polish. It has a blue cover. On the front page of the cover, there is an image of a crowned eagle and document title below: “LICENCE | TO PRACTISE | THE PROFESSION | OF A MEDICAL DOCTOR”. There is numbering on pages 2 to 20. The first page holds the text of the doctor’s oath. The pages 2,3, 8, and 11 are stamped with stamps of the District Medical Council.
