The booklet has the cardboard cover. The inscription "KSIĄŻECZKA WKŁADKOWA" (INSERT BOOK) is in the centre of the cover, below it is a serial number, in the upper left corner a sticker with the inscription "liquidation of PKO in Lviv" and the number 0817. On the second side of the cover a quotation from Ignacy Moscicki. On back cover indistinct pencil inscriptions. At the end of the booklet, there are five tear-off deposit slips.
