Retiree/pensioner ID card belonging to Marta Olchawa (born Bauer) – print on a bifolio, filled in by hand, in Polish. On the first page of the document, in the top half, there is the document name: “RETIREE/PENSIONER | ID CARD | Gives healthcare services | coverage”. Below, there is a guidance note which continues on page 4. Pages 2 and 3 include information on: full name of the holder, date of birth, pension/annuity number, as well as two stamps of the Social Insurance Company, Division I in Warsaw and the name stamp of Elżbieta Piętka with her hand signature.
