State Prize for Julian Stryjkowski for his achievements in science, technological progress, and arts: a full set (badge, badge box, certificate in a cover, certificate holder).

The certificate is enclosed in a red holder with an embossed White Eagle in the upper part. The cover is red, with a golden White Eagle on the front. The certificate is secured under a ribbon attached to the inside of the back cover. It is printed on mouldmade paper with deckle edges. At the top of the page, there is a White Eagle inscribed in a circle on red background. At the bottom, there is an embossed seal with the White Eagle and text: PRIME MINISTER.
