Service card no. 25, folded, with photograph, originally fixed to the paper with staples, issued on 6 June 1945 by the Provincial Jewish Committee in Warsaw - Warsaw-Praga ( 44 Targowa Street) - for clerk Halina Bałucka; three round stamps of the Committee; signatures: Żelazo (green) and St. [?] Waszer (black); on the back further extensions, to 1946, with signatures (two identical ones, both illegible).
